In most cases, the head of household will be the applicant. In the case of an adult with a severe disability, a legal guardian or a relative who functions as a caretaker can apply for assistance.

HOME-New Mexico's application consists of basic information about the family in need of assistance. The application includes a Certificate of Disability that needs to be signed by a professional who is qualified to assess a person's disability (i.e. doctor or case manager).
